If you use the appointment request form, we collect the name, email, phone number, service of interest and message you provide. We use this solely to respond to your request and arrange your appointment. The lawful basis is your consent and the steps necessary to provide the care you have asked for.
The contact form is not a secure channel for health information. Please do not include symptoms, diagnoses or other clinical details in your message — simply ask us to contact you, and we will discuss your needs privately.
This website does not use advertising or analytics cookies, and does not profile visitors. We store a single preference in your browser to remember whether you are reading in English or Portuguese. Fonts are served from our own server, so no request is made to third parties. The map on our contact page is provided by OpenStreetMap, a non-profit project that does not use cookies or track visitors.
Appointment requests are kept only as long as necessary to deal with your enquiry and any resulting care, after which they are deleted or retained under the legal retention periods applicable to clinical records.
You have the right to access, correct, delete, restrict or object to the processing of your personal data, and to data portability. To exercise any of these rights, contact us at clinic@vitality-care.org. You also have the right to lodge a complaint with the Portuguese data protection authority, CNPD (www.cnpd.pt).
